Mental Health Case Management

Adult Mental Health Targeted Case Management Services assist adults with a serious and persistent mental illness (SPMI) to identify their mental health and other service needs, develop their personal goals, and access needed services including:

Eligibility

Eligibility is based on a Mental Health Professional’s determination that the individual needs the service and meets the State’s requirements as having a serious and persistent mental illness (SPMI). A diagnostic assessment is necessary from a Mental Health Professional to establish criteria and the individual needs to be on or eligible for Medical Assistance.

The individual must have a mental illness and meet at least one of the following criteria:
